Logistics Transition Specialist

Patient Engagement Advisors (PEA) is seeking a full‑time Logistics Transition Specialist to work onsite at AdventHealth Zephyrhills and in the local area while performing logistics courier duties. Schedule : Monday – Friday, 9:00 AM – 5:30 PM onsite at AdventHealth Zephyrhills. PEA will provide a vehicle for use of logistic courier duties only. Purpose The Logistics Transition Specialist plays a vital role in achieving the goals of the vision/mission of PEA. To accomplish these goals, the Transition Specialist must exhibit empathy, innovation, collaboration, teamwork, passion, dedication to personal development, and a strong commitment to “know the patient”. The Specialist encourages patients to play an active role in their care experience during and after the hospital stay, ensuring patients understand discharge requirements, are educated on medications, follow‑up care, nutrition, transportation, and assistance programs, and are equipped with the resources necessary for the next stage of care. The Specialist delivers medications, nutrition, product kits, and other patient‑care items securely within the hospital or to the patient’s home or another location. Access to Protected Health Information (PHI) is required. Team members must remain compliant with all confidentiality and security policies and procedures related to PHI. Responsibilities Secure delivery of medications, nutrition kits, and other patient‑care items within the hospital or to the patient’s home or another hospital location. Offer PEA services based on individual patient needs (scheduled appointments, transportation, nutrition, medications, products). Collect and document social determinants of health information for every patient to better understand needs and provide optimal services. Utilize Patient Navigation System (PNS) and other applications to document completed activities and fulfill patient needs. Acknowledge all PNS patient indicators (high risk, payer type, etc.) and coordinate action steps for improved outcomes. Meet KPI “Scheduled” goals and ensure follow‑up appointments are documented in PNS. Collect payments and comply with payment‑collection policies and procedures. Utilize PEA software and assistance programs (coupons, vouchers, discount cards) to provide optimal savings/value for patients. Take action to facilitate generosity services, including leveraging partnerships with community resources. Ensure all patients are offered program services; use missed‑patient reporting to provide services via phone call; provide exceptional care to every patient. Actively participate in hospital‑specific rounds/huddles to obtain critical information and contribute as a collaborative member of the clinical care team/hospitalists. Execute daily workflow procedures to ensure KPI thresholds are met/exceeded; track, engage, and serve expected number of patients. Provide pick‑up and delivery of prescriptions and medical supplies accurately and timely. Ensure proper handling and storage of all medications and supplies. Obtain signature of person receiving the delivery. Meet productivity goals as outlined by site leadership. Submit “Wow Stories” as requested by PEA leadership to the supervisor describing exceptional patient care experiences. Pharmacy Operations / Logistics Courier Work with prescribing clinicians and pharmacists to ensure affordability and successful delivery of medications per Standard Operating Procedures. Continuing Education & Compliance Demonstrate compliance with PEA and hospital‑specific policies and procedures. Maintain a working knowledge of specified equipment, services, and supplies available to transitioning patients. Complete ongoing health system and PEA‑required training. Maintain compliance with all PEA and health system security and privacy (physical and electronic) policies in all work settings (onsite, remote, virtual). Qualifications and Requirements Education and Experience High school education or equivalent required. Must be 18 years old or older. Six months of pharmacy technician experience or one year of medical assistant or other healthcare/medical experience required. Retail pharmacy experience strongly preferred. Other related hospital experience will be considered. Associate or Bachelor’s degree considered in the absence of healthcare experience. Valid driver’s license in the state of employment with a clean driving record. Knowledge, Skills, and Abilities Maintain a clean driving record per company policy. Work a flexible and/or rotating schedule, including nights, weekends, and holidays; may be required to work during inclement weather. Excellent interpersonal skills; connect with patients/families and multidisciplinary providers to build trust and collaboration. Excellent communication skills to facilitate patient understanding and engagement. Manage multiple, complex patients and address diverse needs. Participate in a culture that embraces creativity, productivity, communication, professionalism, and learning. Work in a fast‑paced, rapidly changing environment. Creative problem‑solving, sound judgment, and decision‑making skills. Maintain a high level of professionalism and positively reflect the PEA brand at all times. Computer Skills Web‑based tools (including point of sale, pharmacy information systems, and clinical/EMR systems). Utilize and navigate applications on an iPad. Salary and Benefits Salary range: $20.50–$22.00 per hour.
