Seven Healthcare are seeking an experienced Physical Therapist for a travel contract in Morgantown, WV. This role involves working within a supportive environment, providing comprehensive physical therapy treatments based on physician referrals.

Key Responsibilities of the Physical Therapist

Perform physical therapy evaluations and treatments based on physician orders/referrals
Complete a comprehensive evaluation using appropriate tools and procedures
Review patient records for current diagnosis, history, precautions, and contraindications
Establish measurable goals and develop a treatment plan
Regularly reassess treatment plan effectiveness and patient progress
Communicate patient needs and progress with the treatment team and family members
Consult with medical providers regarding patient evaluation, treatment, and discharge
Recommend and assist in securing adaptive equipment for patients
Maintain and submit evaluation, treatment, and billing documentation
Adhere to standards of the Company's policies and compliance programs
Supervise Physical Therapist Assistants and Aides
Participate in staff development programs and pursue continuing education

Requirements of the Physical Therapist

Valid WV PT license
Experience with home health and OASIS
HCHB experience preferred
