Job Description About Us: Halcyon Home, a trusted and locally-owned home health and hospice company in Austin, is currently seeking a dedicated and experienced Hospice Social Worker to join our compassionate team. This is a PRN position based in our San Marcos office, with a rotating on-call weekend once every 5-7 weeks. The ideal candidate must be licensed as an LMSW, have at least one year of hospice care experience, and possess a deep passion for working with the elderly. Conduct comprehensive assessments of patients' social and emotional needs Develop personalized care plans in collaboration with the interdisciplinary team Provide counseling and support to patients and families facing end-of-life challenges Assist with coordination of care and access to community resources Maintain detailed and accurate documentation of patient interactions and progress Utilize Homecare Homebase EMR (knowledge of which is a plus) Benefits Flexible schedule with Monday to Friday workdays On-call rotations with weekend work as needed Mileage reimbursement also paid at .53/mile Qualifications Licensed as an LMSW Minimum of one year of hospice care experience Minimum of one year of social work experience Strong communication and interpersonal skills Compassionate and empathetic approach to working with the elderly Ability to manage a fast-paced, independent work schedule Knowledge of Homecare Home Base EMR is a plus Medical Specialty Geriatrics Schedule Choose your own hours Monday to Friday Weekends as needed Education Master's (Required) Experience Hospice: 1 year (Required) Social work: 1 year (Required) License/Certification LMSW license (Required) Work Location In person #J-18808-Ljbffr
