Clinical Pharmacist

Position Summary The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry‑level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Responsibilities Determine suitability of individual patients for home care. Collect and organize all patient‑specific information needed to determine suitability for home care. Assess patients’ suitability based on medical condition, prescribed medication therapy, and prognosis. Design, recommend, monitor and evaluate patient‑specific pharmacy care plans. Make recommendations for modifications in prescribed medication therapy or monitoring parameters. Collect and organize patient‑specific information to prevent, detect, and resolve medication‑related problems. Identify appropriate monitoring parameters for each patient. Complete nutritional assessments and make appropriate recommendations. Modify pharmacy care plans based on monitoring data and patient information. Apply pharmacokinetic principles, assess laboratory values, and provide medication‑use education to staff, patients and caregivers. Ensure continuity of pharmaceutical care and communicate pertinent patient information systematically. Prepare and dispense medications following policies, calibrate equipment, and ensure correct concentration and stability. Adhere to safety, quality assurance, labeling, and regulatory requirements (DEA, USP). Manage medication administration equipment and participate in incident reporting. Complete QAR documents, monitor discrepancies in inventory systems, and respond to drug information requests. Attend CEU opportunities and comply with state requirements. Utilize pharmacy support personnel effectively, prioritize workload, and maintain accuracy. Maintain confidentiality of patient and proprietary information and comply with legal and ethical guidelines. Education and Experience Requirements Graduate of an accredited School of Pharmacy. Current, active pharmacy license issued by the State Board of Pharmacy. One of the following: (a) Minimum one year of experience in sterile IV admixture procedures; (b) Knowledge of non‑sterile compounding, immunizations, and medication therapy management; (c) Completion of an ASHP‑accredited Pharmacy Residency Program; (d) Completion of an Advanced Pharmacy Practice Experience; or (e) Equivalent experience approved by the VP of Pharmacy or SVP of Clinical Services. Qualifications Experience providing customer service and meeting quality standards for services. Basic proficiency in Microsoft Word, Excel, and PowerPoint. Willingness to travel up to 5% of the time for business purposes within state. Preferred qualifications: Completion of ASHP‑accredited Pharmacy Residency Program, experience in home care, hospital or clean‑room setting, and at least one year of indirect management experience.
