RN MDS Coordinator – Skilled Nursing Facility (SNF) Location: Boynton Beach, FL Schedule: Full-Time Setting: Skilled Nursing Facility / Long-Term Care Join a Team That Values Clinical Excellence We are seeking an experienced RN MDS Coordinator to join our skilled nursing team in the Boynton Beach area. This role is ideal for an organized, detail-oriented nurse who thrives in long-term care and is passionate about ensuring accurate resident assessments, quality outcomes, and regulatory compliance. Responsibilities Coordinate and complete MDS assessments in accordance with federal and state regulations. Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process. Collaborate with nursing, therapy, social services, and other departments to develop and update care plans. Monitor Medicare and reimbursement documentation requirements. Maintain compliance with CMS guidelines and facility policies. Participate in quality improvement initiatives and survey readiness activities. Educate staff on MDS processes and documentation standards. Qualifications Active Florida RN license required. Previous MDS Coordinator experience in a Skilled Nursing Facility preferred. Strong knowledge of RAI, Medicare, and CMS regulations. Experience with care planning and interdisciplinary team collaboration. Excellent communication, organization, and documentation skills. RAC-CT certification is a plus.
