Opportunity Overview

Our organization is looking for a Nurse Practitioner to become a part of our Kidney Transplant Program located in San Francisco.

This is a full-time outpatient position focused on delivering comprehensive care and ongoing support for kidney transplant recipients, donors, and candidates, working alongside a multidisciplinary transplant team.

You will be joining a skilled and collaborative group committed to enhancing transplant outcomes through evidence-based practices, patient education, and compassionate, team-oriented care.

Position Responsibilities

• Engage in an outpatient role with no inpatient or on-call duties .
• Oversee the care of kidney transplant recipients, including managing immunosuppressive therapy, follow-up care, and coordination of specialty services.
• Conduct pre-transplant assessments and support patient education for both recipients and living donors.
• Work closely with transplant surgeons, nephrologists, pharmacists, dietitians, and social workers to ensure integrated, patient-focused care.
• Participate in quality improvement projects and contribute to program development.
• Ensure timely and precise documentation in the electronic health record system.

Qualifications and Requirements

Required:

• Active California Registered Nurse (RN) and Nurse Practitioner (NP) licenses.
• Master’s degree in Nursing from an accredited institution.
• National NP certification (ANCC or AANP).
• Current DEA, BLS, and ACLS certifications.

Preferred:

• At least 2 years of experience as an NP in transplant, nephrology, internal medicine, or surgical specialties.
• Experience managing kidney transplant or solid organ transplant patients is highly desirable.
• Excellent communication, collaboration, and critical thinking abilities.
