Physical Therapist - Travel Contract

Join our team as a traveling Physical Therapist in Mentor, OH, where you will help older adults recover from injuries and manage chronic conditions in a skilled nursing facility setting. This opportunity involves working with geriatric patients in a facility that typically maintains 100–110 residents and uses NetHealth/PCC for electronic medical records. Minimum Requirements Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Ohio physical therapy license required to start the assignment. BLS certification from American Heart Association or American Red Cross may be required. Experience working with older adults/geriatrics in skilled nursing or long-term care environments preferred. Experience using NetHealth/PCC or similar skilled nursing EMR preferred. Preferred Qualifications Prior experience in a 100+ bed skilled nursing facility preferred. Experience with restorative nursing programs and fall prevention initiatives preferred. Assignment Details Facility uses NetHealth/PCC electronic medical record system. Assignment location: Mentor, OH (Cleveland metro area). Coordinate with interdisciplinary teams, document interventions in the EMR, and contribute to restorative and discharge planning efforts.
