Cook Hospital is seeking a Full Time Staff Physical Therapist to join our Rehabilitation Team at our facility in Cook, Minnesota.

• Sign on Bonus: $7000

Responsibilities


The staff Physical Therapist is directly responsible to the Directorand/or his or her designee for direct and non-direct patient care issues. The staff Physical Therapist has theresponsibility for direction, utilization, supervision, instruction andevaluation of professional and supportive staff, volunteers and students intheir area of the physical therapy department/facility.

To accomplish this job, an individual must be able to perform each essential function satisfactorily.

1. Scheduling patients for treatment and evaluations.
2. Preparing reports and records by department/facility personnel.
3. To inform appropriate personnel of inventory of materials and supplies for their area of the department/facility.
4. Utilizing skills of volunteers who may assist in their area.
5. Utilizing skills of Student Physical Therapist and Physical Therapist Assistants and Physical Therapists.
6. Utilizing skills of Rehab Secretary and Rehab Aide appropriately.

B. Evaluations:

1. To be responsible for the evaluation of patients referred for physical therapy.
2. To be responsible for completion and submission of all reports pertaining to the evaluation in a timely manner, 24 hours or less.
3. To see that all referral sources are advised of department/facility action with the case referred.

C. Treatment:

1. To practice Physical Therapy consistent with the American Physical Therapy Association Code of Ethics and Standards of Practice policies.
2. To maintain a patient treatment schedule consistent with the needs of the department/facility.
3. To confer with the person(s) most closely associated with the active management of the patient and to keep them informed of any additional patient needs, treatment modification and/or progress.
4. To treat patients and function as a Staff Physical Therapist as necessary.
5. To treat patients of any age (e.g. infants, children, adolescents, adults, geriatrics, elderly, ages 1-120 years), race, creed, gender and disability without bias or prejudice.
6. To respect and understand the patient's right to treatment and right to involvement in treatment decisions.
7. To practice ethical responsibility based on the cultural, ethnic and religious beliefs of the patients served and applicable law.

D. Records andReports:

1. As per facility guidelines to be responsible for the authentication, preparation and submission of department/facility reports, ensuring confidentiality of all records, including:
   1. Evaluation reports/summaries
   2. Discharge reports/summaries
   3. Letters of medical necessity
   4. Progress reports to referring physicians and agencies
   5. Current progress notes on all patients within their area
   6. Special reports as requested by the Director.
   7. Daily attendance records and summaries of same.

E. Meetings andConferences

1. To attend and participate in Department/Facility Meetings as required
2. To attend regular department/facility meetings to:
   1. Report on and discuss activities and needs of department/facility
   2. Discuss patient's needs and progress
   3. Discuss reports and records
   4. Discuss expansion in terms of future goals and needs

F. ProfessionalStatus and Public Relations:

1. To keep abreast of new techniques and trends in Physical Therapy.
2. To attend conventions, meetings or special courses whenever possible and to share with the department/facility personnel information gained thereby.
3. To prepare and give talks to community groups as requested
4. To summarize any continuing education course and submit an article to local papers as a news release.
5. To maintain good public relations with all community sources.
6. To communicate in a manner consistent with the Cook Hospital's "Commitment to co-workers" as to foster a pleasant work environment and team approach to patient care .

Licensure/Certification/Registration: Must have, orbe eligible for, State Licensure or Registration. Registration must be obtained within oneyear, or as per state law. If not registered, must have temporaryregistration. Must have verification card of currentinstruction in CPR.


Education: Graduateof and have a certificate from an American Physical Therapy Association - approved school of Physical Therapy,or a school approved by the World Congress of PhysicalTherapy.

Experience: Preferred minimum of twoyears experience.
