Work Shift The physical therapist will work part‑time on a flexible schedule to meet patient needs and clinic requirements. Job Summary The physical therapist assesses, plans, organizes and participates in rehabilitative programs that improve mobility, relieve pain, increase strength and prevent deformity for patients with disease or injury. Services are delivered across the full continuum in a cost‑conscious, quality‑focused, customer‑oriented manner while complying with all regulatory standards. Core Responsibilities and Essential Functions Treatment Planning and Provision of Care Implement the physical therapy treatment plan. Select appropriate treatment activities to progress patients toward goals. Revise goals and plan of care with patient/family input. Provide ongoing patient/family education and training. Provide discharge instructions, follow‑up, and referral to community resources as appropriate. Act as a referral source for staff, families, and physicians on services and equipment related to rehabilitation services. Patient Assessment Collaborate with the physician to establish and evaluate treatment programs, plan of care, goals, and discharge plans related to the patient’s physical therapy needs. Establish a plan of care in conjunction with the patient and/or family members. Develop a realistic discharge plan incorporating patient/family goals. Documentation Document evaluation and plan of care according to departmental guidelines. Record each treatment session per departmental protocols. Complete and submit family education, discharge, and other required documentation within facility and program guidelines. Organize workload to complete responsibilities in an appropriate and timely manner. Utilize materials and equipment safely and cost‑effectively. Observe infection prevention precautions. Maintain required educational credentials to retain licensure. Participate in orientation and training of new employees. Complete all mandatory hospital/system requirements. Perform other duties as assigned. Comply with all Wellstar Health System policies, standards of work, and code of conduct. Required Minimum Education Bachelor’s in Physical Therapy, Master’s in Physical Therapy, or Doctorate in Physical Therapy. Required Minimum License(s) and Certification(s) PT – Physical Therapist. BLS – Basic Life Support, or ARC‑BLS, or BLS‑I. Required Minimum Skills Effective communication skills. Customer‑service focus. Team orientation. Basic computer skills.
