Home Care SupervisorLocation: Homestead, PASchedule: 7:30a-4p M-F with on call rotation + as needed based on participant careSummary of Job Duties: The Home Care Supervisor (HCS) will provide consistent supervision to Community Care Aides using effective communication and guidance. This person will strengthen the relationship between the Home Care Department and the Inter Disciplinary Team (IDT). This person will provide hands on leadership in all aspects of home care. The HCS will develop partnerships with families and caregivers. The HCS may be required to make in-home visits as necessary for assessment and on-going case management. Experience teaching patients and other health care workers preferred.Required Certifications/Licensure: Valid Pennsylvania COTA/L, PTA, CNA or LPN license.
