Salary Range: $42,000-$47,000
Program Description
The 59th Street Women’s Shelter is a 200-bed, 24/7 emergency shelter for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, occupational therapy, and housing placement assistance. Additionally, a street-facing, on-site Article-28 Clinic provides primary care, and behavioral health care services.
Overall Responsibilities
Under the general direction of the Director of Social Services and Clinical Coordinator, the Intake Case Manager is responsible for providing required assessments, case management, and providing delivery of service to the client; these responsibilities will ultimately assist the client in addressing their dependencies and assume control of their lives to eventually live independently and return to permanent or other alternative housing in their communities. The case manager is also responsible for providing referral services to the clients on their caseload and assisting with other cases as need. The case manager will ensure that proper protocols are being followed and will ensure that chart(s) remain audit ready at all time; performs related work.
Essential Duties and Responsibilities
The essential duties of the Intake Case Manager include but are not limited to the following activities:
Outreaches all new clients to complete the 59th Street Intake Packages and create a physical client chart within 48 hours of a client’s entry to the shelter to meet funder requirements
Carries a reduced caseload of new clients for a minimum of 2 weeks and completes the DHS Initial Shelter Assessment and Independent Living Plan
Makes necessary referrals for services such as psychiatric evaluation, physical, PPD etc…
Provides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long-term objectives of which can be achieved
Inputs all client information and updates in a timely fashion into the CARES system as well as the client’s physical chart
Works collaboratively with clients and other staff to identify and create a plan to identify and obtain financial entitlements, housing and other concrete services
Maintains accurate and up to date written client documentation as required and in accordance with City/State oversight agency guidelines
Maintains liaison with representatives from community organizations
Provides escort services to clients’ appointments as needed
Attends and participates in program meetings
Performs other duties as assigned by the Clinical Coordinator, Director of Social Services and/or Program Director
Physical Activities
While performing the duties of this position, the Intake Case Manager must be able to navigate the 9-story facility by foot, climbing up and down the stairwells, in order to observe staff and clients. The Case Manager must be able to stand and bend in order to review client files, put them away, and close them out. It is also part of the role to sit at the computer workstation for extended periods to review the client data and update the information in the case management systems. The Case Manager must be able to lift or move material and equipment weighing up to 15 pounds.
Education and Experience Requirements
A Bachelor’s degree in social work, public administration, psychology, education, sociology, occupational or physical therapy, or other related human services fields with a minimum of two (2) years of experience
An Associate’s degree in the aforementioned fields with three (3) years of experience
A High School Diploma or GED equivalent with four (4) years of experience
Bilingual or multilingual a plus
Preferred Skills
Leadership: Demonstrated ability to manage and motivate staff to accomplish stated program goals and objectives while developing their individual and group skills.
Team building: Ability to develop and foster, through demonstration, a team spirit, enabling staff to overcome the challenges of providing needed services to a special client population.
Communication: Excellent oral, writing, and listening skills are a must.
Organizational: The ability to work well within a highly pressurized, deadline environment, while meeting the short-term and long-term mandates of the program.
Interpersonal: An ability to interface with clients as well as all levels of staff.
Computer: Knowledge of case management software as well as proficiency in Microsoft Office Suite
