Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinator to join our CLTS Waiver Program team in Waukesha County! The Children’s Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to thrive in home and community settings. As a Care Coordinator, you’ll be the primary point of contact for families, helping them access and navigate essential services. Please note: This is a continuous posting in anticipation of future growth opportunities in the Waukesha County area.
WHAT YOU’LL DO
- Assess children’s functional abilities using approved tools
- Develop and implement individualized service plans with families and
- Maintain accurate documentation and meet all regulatory timelines
- Collaborate with internal teams and external agencies to support families
- Testify in legal proceedings when required
- Communicate clearly with families, providers, and team members
- Participate in staff development, training, and supervision

QUALIFICATIONS
* Bachelor’s degree in a human services field (e.g., Social Work, Psychology, Special Education, Counseling, etc.)- Minimum 1 year of experience working with children with disabilities
- Bilingual fluency in Spanish is preferred but not required
- Proficient in computer systems and electronic health records
- Strong interpersonal and organizational skills
- Valid driver’s license and reliable transportation (MVR check required)
WORK ENVIRONMENT
- Community-based with daily travel required
- Moderate noise level; occasional exposure to outdoor conditions
- Flexibility to meet family needs, including crisis response
