Remote Care Manager for Mental Health & Addiction Support
Home State Health
Centene in Missouri is seeking a Care Management professional focused on mental and behavioral health to develop, assess and facilitate complex care plans for members and families, delivering education and personalized support to improve outcomes.
The role allows remote work from within Missouri and requires a master's level BH credential or nursing degree with BH experience, plus 2–4 years of related practice. Monday–Friday 8am–5pm, with potential field visits as needed.
