Responsibilities:

• Conduct initial evaluations and develop individualized treatment plans for patients with a variety of medical conditions and injuries

• Provide physical therapy services, including exercises, stretches, and modalities, to promote patient recovery and independence

• Collaborate with interdisciplinary healthcare team members to develop comprehensive treatment plans and ensure patient-centered care

• Monitor patient progress, adjust treatment plans as needed, and report significant changes or concerns to the healthcare team

• Maintain accurate and detailed records of patient treatment and progress

• Participate in patient education and discharge planning to ensure patients are equipped with the skills and knowledge necessary for successful rehabilitation and independence

• Current licensure as a Physical Therapist in the state of California
