Assertive Community Treatment Teams facilitate group and individual mental health services with adults. Focusing on psychoeducation, affordable housing, employment, and psychiatric services MHP’s assist individuals in remaining out of inpatient care setting by assisting in maintaining stability within their communities Required Education/Experience Graduate Degree in psychiatry, psychology, social work, nursing, education, rehabilitation, or any graduate degree in the field of human services Registered Nurse with five years’ experience in the mental health field, plus have certification by the Department of Public Welfare, Office of Mental Health (OMH), as a mental health family-based worker Experience Minimum of 1 year of clinical or rehabilitation experience working with adults with severe and persistent mental illness, internship and/or practicum experience are considered experience if conducted in a clinical inpatient or outpatient setting Rate: 26 + based on experience Required Licenses/Certifications Valid driver’s license Responsibilities Develops and maintains positive relationships with key stakeholders. Ensures stakeholder satisfaction with the company by maintaining proactive, responsive communication. Maintains regular, proactive communication with key stakeholder groups, and builds new relationships as needed to further the company’s mission and vision. Develop meaningful relationships and collaborative with external resources for community linkage and aftercare planning. Establishes and maintains frequent and regular communication with Team Leader and Team Assistant. Report treatment difficulties to supervisor. Identifies and executes on growth opportunities. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess strong interpersonal skills to establish and maintain effective working relationships with client, child, parent/guardian, treatment team members, and other personnel involved in treatment. Must have knowledge of various therapy techniques and counseling strategies applicable to serving members with persistent mental health challenges. Must have knowledge common behavioral disorders exhibited by adults with persistent mental health challenges (specifically Bipolar, schizophrenia, schizoaffective disorder). Must have strong written and verbal communication skills. Must display professional attitude. Must display empathy, active listening skills, critical thinking/problem solving skills. Performs other related duties as assigned. Independent scheduling Training/supervision - Has an authentic interpersonal style that engages others and encourages high performance in Attend trainings as directed by supervisor, in accordance with company policy, and pursuant to applicable regulations. Attend required supervision sessions and staff meetings in accordance with program/company requirements. Provide training to staff and clients regarding Fair Housing Act, American with Disabilities Act, and/or other state or federal legislation regarding safe and affordable housing for persons with disabilities. Communicating Effectively (Supported by Seek First to Understand then to be Understood) - Share’s information. Listens and involves others. Clearly Conveys ideas in a manner that engages others and helps them understand and retain the message. Communicates with other providers and coordinates care regularly with all individuals involved in treatment. Adheres to HIPAA privacy regulations and Department of Human Services/CASSP principal guidelines. Adheres to Mandated Reporter protocol and effectively communicates concerns with program management. Work collaboratively with members of the treatment team and other service providers to ensure comprehensive case management. Managing Stress-(Supported by Sharpening the Saw) Uses healthy and effective methods to manage stress (e.g., maintaining appropriate diet, exercise, and sleep schedules); reacts effectively when faced with high-pressure situations Championing the needs of the People we Support/Customers (Supports Culture of Caring)-Provides timely and professional service to both internal requests and the requests of the people we support and customers; is responsive to the people we support and customers' needs and requests. Following Policies and Procedures- Understands and follows company policies; encourages others to follow work rules; calls attention to actions that may violate policies and procedures. Meeting Basic Work Expectations-Complies with basic job requirements and tasks; arrives at work on time as scheduled; proactively informs others when he/she encounters problems that may limit his/her ability to meet expectations. Provide individual/group therapy treatment in office and in the community, setting using a stage-based model Utilize safe crisis prevention techniques as deemed appropriate. Document treatment data/results that are clinically relevant on progress notes and submit in a timely manner (within 24 hours of each encounter). Complete client intake paperwork/assessments per company/program guidelines Complete recurrent assessments within the regulated timeframes Participate in interdisciplinary team meetings and other miscellaneous meetings, as necessary. Collaborate with other providers to secure appropriate aftercare/discharge services and community linkages supports during the course of treatment (county, housing, MHDS and all other providers involved) Develop, implement, evaluate, and revise treatment goals and plans in collaboration with client and team members in accordance with regulated timeframes. Assist clients in finding safe and affordable housing Coordinate detoxification services with external inpatient service providers and establish linkage to self-help programs as needed. Assist clients in budgeting and daily living skills to maintain stability in home and community settings. Transport clients when necessary Pre-employment screening: Complete criminal background Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities) Drug testing Education verification and other credentialing based on position requirements.
