Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

Deliver advanced ambulatory clinical pharmacy services as a credentialed Clinical Pharmacy Specialist under Collaborative Practice Agreements (CPAs). This role focuses on optimizing therapeutic outcomes, improving medication access, and reducing healthcare costs within an established ambulatory care network. By partnering with multidisciplinary teams, including prescribers and nurses, you will design, implement, and monitor evidence-based therapeutic plans aligned with Comprehensive Medication Management (CMM) standards.

Key Responsibilities:

• Provide direct ambulatory clinical pharmacy services, including assessment, monitoring, and documentation of patient therapeutic plans under pharmacy CPAs.
• Conduct patient visits encompassing medication reconciliation, laboratory monitoring, disease state management, and counseling.
• Collaborate with healthcare providers to implement medication interventions supporting quality, safety, and cost-effectiveness.
• Manage referrals with closed-loop communication to ensure coordinated care.
• Facilitate patient access through prior authorization coordination, financial assistance programs, and benefit investigations.
• Oversee specialty medication therapy, ensuring safe initiation, monitoring, and continuity.
• Deliver drug information and education to patients, families, and care teams.
• Precept pharmacy students, interns, and residents as assigned.
• Engage in system improvements, policy development, research, and quality initiatives to enhance ambulatory pharmacy services.
• Document all interventions and report outcomes according to organizational and national standards.

Qualifications:

• Active Delaware Pharmacist License in good standing (or willing to obtain promptly).
• Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (required).
• Minimum one year of clinical pharmacy experience, including residency.
• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD strongly preferred).
• Experience with Collaborative Practice Agreements, specialty medication management, and CMM preferred.
• Preceptor experience and prior authorization expertise are advantages.
• Completion of PGY1 or PGY2 ambulatory care residency is a strong plus.
