Observe, document, and promptly report changes in patient condition to the supervising nurse. Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life. Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment. Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase). Qualifications Active Certified Nursing Assistant (CNA) certification Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred. Field Role Requirements Valid driver's license, reliable transportation, and current automobile insurance.
