Medical Assistant Care CoordinatorWe are seeking a dedicated and compassionate Medical Assistant Care Coordinator to join our team. This role supports our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, focusing on improving patient compliance, coordinating care, monitoring health metrics, and reducing unnecessary hospital and urgent care visits.ResponsibilitiesConduct monthly CCM and RPM patient outreach calls.Review medications, treatment plans, and patient concerns.Monitor blood pressure readings and respond to RPM alerts.Coordinate care between patients, providers, pharmacies, and medical equipment vendors.Assist with obtaining and facilitating physician orders for durable medical equipment.Document patient interactions and maintain compliance within the CCM/RPM platform.Ensure Medicare-required patient engagement time is achieved and documented.Educate patients and provide support regarding chronic health conditions.Identify barriers to care and escalate concerns to providers when necessary.Essential SkillsMedical Assistant certificateStrong interpersonal and communication skillsWarm, engaging, and personable demeanorAbility to build rapport and comfortably converse with elderly and chronically ill patientsAdditional Skills & QualificationsPrior CCM (Chronic Care Management) experiencePrior RPM (Remote Patient Monitoring) experienceExperience coordinating patient care, medication management, or follow-up careExperience in a physician practice, primary care, or care management settingWork EnvironmentThis position is office-based, working onsite Monday through Friday from 9:00 AM to 5:00 PM in Queens, NY. You will be part of a team currently consisting of three medical assistants, with plans to add two more. The role focuses on patient interactions and coordination over the phone.Job Type & LocationThis is a Permanent position based out of Queens, NY.Pay and BenefitsThe pay range for this position is $41600.00 - $52000.00/yr.
