Certified Nurse Aide Home Health Aide

BrightCare HomeCare, a leading provider of home care services in Louisiana, is seeking a compassionate and skilled Certified Nurse Aide Home Health Aide to join our team of dedicated professionals. As a Certified Nurse Aide Home Health Aide, you will have the opportunity to provide high-quality care to individuals in the comfort of their own homes, helping them to achieve optimum health and wellness.

As a Certified Nurse Aide Home Health Aide with BrightCare HomeCare, you will be part of a dynamic and compassionate team committed to delivering exceptional care services to our clients. Responsibilities:

Provide personalized care services to clients, including personal care, assistance with daily living activities, and support with mobility and transfers
Conduct vital signs and report any changes to the nurse or supervisor
Assist with medication management, including administration and documentation
Collaborate with the healthcare team to develop and implement individualized care plans
Maintain accurate and detailed records of client care and services provided
Communicate effectively with clients, families, and healthcare providers to ensure seamless care coordination
Participate in in-service training and education to enhance skills and knowledge

Working Conditions:

Work in clients' homes, with varying schedules and environments
May be required to lift, transfer, or reposition clients, and move equipment or supplies
Must be able to work independently and as part of a team
Must be able to effectively communicate with clients, families, and healthcare providers
