Hybrid LTSS Case Manager - Home & Facility Visits

UnitedHealthcare is seeking a Long Term Services and Supports (LTSS) Case Manager to support case management goals and participate in case conferences with the Supervisor. You will help coordinate care, document contacts, and monitor member health and welfare in a compliant system. The role requires about 50% travel to members’ homes or long-term care facilities. You will work in Delta, CO with a hybrid schedule, visiting the Delta office two days per week, and maintain strict confidentiality #J-18808-Ljbffr Inspire Health Alliance
