RN Case Management Manager — Discharge & Transitions

UnitedHealth Group is seeking an experienced RN Case Manager to lead the NV Medical Management Command Center, overseeing discharge planning, transitions of care and utilization management for high‑risk patients in acute and sub‑acute settings. This leadership role requires a Nevada RN license, CCM certification within 2 years, 1+ year supervisory experience, strong regulatory knowledge, technology skills and exceptional communication. #J-18808-Ljbffr RXinsider LTD.
