Job Summary:

Responsibilities:

• 
  Administer medication and treatments accurately according to physicians orders.
  
  
• 
  Monitor and assess residents health status, reporting any changes promptly.
  
  
• 
  Document patient information thoroughly in compliance with facility protocols.
  
  
• 
  Collaborate with the healthcare team to develop and implement care plans.
  
  
• 
  Ensure infection control protocols are followed consistently.
  
  
• 
  Assist residents with activities of daily living to promote comfort and dignity.
  
  

Qualifications:

• 
  Valid LVN or LPN license in the state of employment.
  
  
• 
  Proven experience working in LTC or SNF environments preferred.
  
  
• 
  Must have completed the initial or updated COVID-19 vaccination or a valid medical/religious exemption.
  
  
• 
  Strong clinical assessment and documentation skills.
  
  
• 
  Ability to work flexible schedules, including Wednesdays, Thursdays, and alternating weekends.
  
  
• 
  Excellent communication and compassionate patient care skills.
