Responsibilities:

• Provide direct patient care, including vital sign monitoring, baths, dressing, grooming, and toileting assistance
• Assist with activities of daily living (ADLs) such as feeding, incontinence care, and transfers
• Maintain accurate and detailed records of patient care and condition
• Collaborate with the healthcare team to develop and implement individualized care plans
• Provide emotional support and companionship to residents
• Perform routine tasks such as answering call lights, taking charts, and distributing medications
