Licensed Practical Nurse - Pediatrics

First Choice Health Centers (FCHC) is a Federally Qualified Health Center (FQHC) located in East Hartford, CT, offering outpatient nursing opportunities. Staff LPNs primarily manage urgent and emergent patient calls across all disciplines and perform licensed professional nursing functions to assess patient needs and severity. The LPN facilitates patient medication refills, manages providers’ schedules, counsels patients, and performs additional tasks as needed. Pay is based on factors such as work experience and certifications. Minimum Knowledge, Skills & Abilities Required Experience with an electronic health record system Vaccine administration experience Excellent communication and interpersonal skills Positive attitude and willingness to be cross‑trained in multiple specialty areas, including pediatrics Experience and Training Valid LPN license in the state of Connecticut BLS certification Standard Job Duties Provide direct and individualized nursing care to patients using the nursing process Maintain accurate medication inventory listings and oversee the CT Vaccine Program, including monthly inventory, ordering, and documentation in the electronic health record Accurately document all assessments, vital signs, specimen collections, interventions, evaluations, and referrals in the proper patient record Manage prescriptions and medication refills for providers according to nurse protocols Perform wound care using appropriate infection control practices Complete meaningful use documentation, including depression screening, sexual history, and substance abuse history Refer appropriate patients to specialty providers based on assessment findings Work with medical assistants to ensure safe and effective patient care within scope of practice Engage with outside agencies to manage patient care outside the Center Monitor task lists (telephone encounters) to respond promptly, follow up on overdue orders, and track critical referrals Adhere to all patient safety standards and infection control guidelines Assist the care team to assure smooth office operation and delivery of excellent service through teamwork COVID‑19 Considerations Employees must be vaccinated against COVID‑19.
