Compensation Structure

• Salary range: $80,000 - $110,000/year Salary Range: 85,000 - 120,000
• Start of Care (SOC): 2.0 Units
• Evaluation / Re-Evaluation / Recertification / Resumption of Care: 1.5 Units
• Routine Visit: 1.0 Unit
• OASIS Discharge: $65

Responsibilities

• Conduct comprehensive assessments of patients’ health needs and develop individualized care plans.
• Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
• Provide education and support to patients and their families regarding health management, disease prevention, and treatment options.
• Perform wound care, catheter care, and other nursing procedures as needed.
• Document patient progress accurately in electronic health records (Epic) and maintain compliance with regulatory standards.
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate care effectively.
• Ensure infection control protocols are followed to maintain a safe environment for patients.
• Participate in ongoing professional development and training opportunities.

Qualifications

• Active Florida RN License
• CPR Certification
• Strong assessment skills
• Home health/OASIS experience preferred but not required
