divh2Internal Medicine Nurse Practitioner/h2pCareOne at Weymouth, in partnership with BridgeCare Provider Group is now hiring an Internal Medicine Nurse Practitioner./ppSalary Range $125,000 to $135,000 annually./ppCompensation will be based on, but not limited to, experience, qualifications, credentials and any other relevant information./ppAdvanced Practice Nurse will be responsible for, but are not limited to:/polliConduct visits to patients in accordance with local, state and federal regulations./liliCommunicate visit findings to designated members of the interdisciplinary team./liliFacilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment)./liliConsult with other members of interdisciplinary team to contribute to the patients plan of care./liliPrepare and maintain accurate patient records, charts and documents./liliAssist in development of clinical practice guidelines/standards in support of quality care as needed./liliEnsure collaboration with patients, caregivers, medical directors and members of IDT./li/olpEducation: Masters degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program. Professional Experience:/pulliMust have a Nurse Practitioners license in the state of operations/liliMust have 1+ years experience working within Family, General Practice, Geriatric care or related field/liliMust have a valid drivers license and a reliable source of transportation/liliMust be willing to travel locally/liliExperience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required./liliCommitment to ongoing professional development and clinical excellence./li/ulpThe CareOne mission is to define excellence within the health care community.
