Director of Case Management Key Responsibilities & Duties Lead and manage the case management department, ensuring efficient operations and compliance. Develop and implement strategic plans for patient care coordination and resource management. Collaborate with healthcare teams to optimize patient care and transitions. Monitor and ensure adherence to state and regulatory requirements. Provide leadership and mentorship to case management staff, fostering a culture of excellence. Analyze data to identify areas for improvement and implement solutions.Maintain up-to-date knowledge of case management trends and practices. Coordinate with social work teams to address patient needs effectively. Director of Case Management Job Requirements Bachelor's Degree required; Master's Degree preferred in Nursing or related field. Minimum of seven years of healthcare experience, with five years in leadership roles. Current NYS Licensure as a Registered Professional Nurse or Social Worker required. Certification in Case Management and PRI Certification preferred. Strong understanding of patient care assessment, planning, and management. Familiarity with state and regulatory requirements related to case management. Proven ability to lead teams and drive performance improvements.
