Overview
Physical Therapist – Home Health in Fort Bragg, California. This travel, temporary assignment starts 06/22/2026 and runs for 13 weeks. Responsibilities
- Assess and treat patients in an outpatient/home health context with a focus on functional outcomes and discharge planning
- Develop, implement, and adjust individualized physical therapy plans of care based on evaluation findings and progress
- Reassess patients regularly, document progress, and communicate results with the interdisciplinary team
- Educate patients and caregivers on therapy plans, safety, and home exercise programs; ensure appropriate referrals when needed
- Maintain accurate documentation in the medical record and comply with licensure, safety, and facility policies
- Collaborate with nursing, rehabilitation staff, and case management to support timely patient discharge and transition of care

Requirements
- Active Physical Therapy license in the state of practice (California)
- BLS certification (Required)
- 1 year of physical therapy experience; 2 years preferred
- Must have worked in the unit/therapy specialty within the last 3 months
- Ability to work in an outpatient/home health setting within a hospital-type environment
