Assistant Director of Nursing (ADON) LPN or RN South Prairie Memory Care | Bethalto, IL

South Prairie Memory Care is seeking a compassionate and motivated Assistant Director of Nursing (ADON) to join our leadership team. This is an excellent opportunity for a nurse who enjoys both direct resident care and clinical leadership.

This role combines hands-on nursing care with administrative responsibilities. The ADON works closely with the Director of Nursing to support resident care, staff development, quality assurance, and regulatory compliance while maintaining a regular clinical assignment.

Position Summary

The Assistant Director of Nursing (ADON) provides direct resident care while assisting the Director of Nursing with clinical operations, staff oversight, regulatory compliance, and quality assurance activities. The ADON serves as a clinical leader and resource for staff, helping ensure residents receive safe, compassionate, and high-quality care.

The ADON is expected to maintain a regular clinical assignment and work alongside caregivers and nursing staff while completing administrative responsibilities delegated by the Director of Nursing. This position combines hands-on resident care with leadership responsibilities to support the overall success of the nursing department.

Essential Duties and Responsibilities

• Provide direct nursing care and medication administration in accordance with resident service plans, physician orders, and facility policies.

• Maintain a regular floor assignment and assist with resident care needs as scheduled.

• Monitor residents for changes in condition and communicate concerns to the Director of Nursing, physicians, and other healthcare providers.

• Assist with admissions, transfers, discharges, assessments, and care plan follow-up as assigned.

• Provide guidance, support, and mentorship to nursing staff, caregivers, and resident assistants.

• Assist the Director of Nursing with daily clinical operations and delegated management responsibilities.

• Conduct chart audits, medication record audits, treatment record audits, and documentation reviews as assigned.

• Assist with investigation and follow-up of incidents, falls, medication errors, resident concerns, and quality assurance activities.

• Assist with staff orientation, training, competency evaluations, and ongoing education.

• Monitor compliance with facility policies, service plans, and clinical documentation requirements.

• Assist with infection prevention and infection control monitoring activities.

• Participate in quality improvement initiatives and regulatory survey preparation.

• Serve as a clinical resource for nursing staff and caregivers.

• Assist with staffing coordination and communication regarding staffing concerns.

Additional Expectations

• Participate in the clinical on-call rotation, which is shared with the Director of Nursing to ensure 24/7 clinical support and availability for staff.

• Respond to staff calls, resident care concerns, changes in condition, and clinical questions during assigned on-call periods.

• Communicate significant resident concerns, emergencies, or operational issues to the Director of Nursing and Executive Director as appropriate.

• Provide floor coverage during staffing shortages, emergencies, vacations, and call-offs.

• Maintain visibility and accessibility to residents, families, and staff through regular presence in resident care areas.

Qualifications

• Current Illinois Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing.

• Minimum of two years of nursing experience.

• Experience in assisted living, memory care, long-term care, or geriatrics preferred.

• Knowledge of dementia care and memory care best practices.

• Strong leadership, communication, and organizational skills.

• Ability to balance direct resident care responsibilities with delegated administrative duties.

• Ability to work independently and as part of a multidisciplinary team.

• CPR certification preferred.
