PRN Pharmacist

Westminster Pharmacy Services 13921 Shell Point Plaza Fort Myers, FL 33908, USA Longevity Scripts is seeking an on‑call Pharmacist for our Ft. Myers location. This position will evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Responsibilities Make recommendations for therapy changes to physicians and facility staff. Conduct Drug Utilization Review (DUR) on all prescriptions and review clinical edits to ensure no adverse drug interaction. Maintain patient/resident database with current and past medical history, laboratory data, and concurrent medication information. Reconcile Controlled Substances logs with inventory on hand and dispensed prescriptions in compliance with state and federal regulations. Provide clinical expertise by responding to drug‑related questions from technicians, nursing staff, directors of nursing, and administrators. Document all communication with physicians, nurses, other staff and patients/residents in the medical record. Work rotating weekends and/or nights; staff must work at least 1 shift per month. Qualifications Doctorate of Pharmacy. 1–3 years of long‑term care (LTC) pharmacy experience. Current licensure in the State of Florida. IV clinical experience preferred. Strong organizational skills and ability to prioritize multiple tasks in a dynamic environment. Equal Opportunity Employer.
