Case Management Coordinator

Position Overview Market leading national healthcare organization seeks a Medical Case Manager in a hybrid remote and field based role. The position requires residence in one of the following cities: Lakeland, Auburndale, Bartow, Lake Wales, Winter Haven. The role offers autonomy with a Monday‑Friday 8:00 AM–5:00 PM schedule and flexibility while coordinating member care. Key Responsibilities Telephonically and/or face‑to‑face assessment, planning, implementation, and coordination of all case management activities with members to evaluate medical needs and facilitate overall wellness. Manage a caseload that includes supportive and medically complex members. Develop proactive courses of action to address issues and enhance both short‑ and long‑term outcomes. Determine appropriate services and supports based on member health needs, including Prior Authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources and supports. This role will require 50‑75% travel for face‑to‑face visits with members in Lakeland, Auburndale, Bartow, Lake Wales, Winter Haven (mileage reimbursed). Qualifications Self‑motivated, energetic, detail‑oriented, highly organized and tech‑savvy Case Management Coordinator. Bilingual fluent in Spanish and English. Ability to multitask, prioritize, and adapt to a fast‑paced changing environment. Effective verbal and written communication skills.
