Job ID View phone number on click.appcast.io JOB OVERVIEW The Wellness Nurse is responsible for monitoring the health and well‑being of our residents and providing leadership support and assistance with resident care and services. Responsibilities include overall well‑being of residents, assisting in training and monitoring of medication administration, communication with resident support services and families, and maintaining clinical quality assurance while demonstrating the Mission for Sunrise Senior Living, “to champion the quality of life for all seniors” in accordance with federal, state/provincial, and local standards and Sunrise Senior Living policies.
RESPONSIBILITIES & QUALIFICATIONS
Essential Duties Coordination of Health Needs Identify ongoing needs and services of residents to promote the highest quality resident services. Ensure proper follow‑through and participate in evaluation and documentation for residents with a change in health care services and status. Assist Resident Care Director (RCD) in conducting resident evaluations and gathering data at move‑in, 30 days post move‑in, semi‑annually, and with any change in status. Conduct monthly wellness visits for all residents. Follow up with residents who have experienced a fall, accident, or incident, document, and report as required. Document all pertinent information in the resident wellness file. Maintain communication with residents’ families or responsible parties to ensure medical needs are met. Contact residents’ attending physicians when necessary or upon family request. Ensure weights and vital signs are obtained monthly for each resident. Assist in maintaining wellness files according to Sunrise policies and regulations. Maintain medical supplies and emergency kits for the community. Provide clinical support and assistance to community team members as needed. Understand and follow infection control practices. Partner with the RCD and community team to promote infection control standards, including resident testing, training, and skill checks. Disposal of fluid and hazardous materials in compliance with universal care precautions. Medication Programs Monitor each resident’s medication and treatment profile to ensure administration as ordered and documented accurately. Transcribe physician orders into medication administration records (MAR/TAR), including initials, date, time, and date faxed to pharmacy accurately and promptly. Complete MAR/TAR audit each month. Demonstrate knowledge of the Six (6) Rights of Medication Pass. Act as liaison for pharmacy services to ensure effective services for residents. Monitor the medication room, medication carts, and treatment carts for neatness, cleanliness, availability, and expired medications. Complete medication observations and skill checks for medication care managers as directed by the RCD. Quality Assurance and Regulatory Compliance Demonstrate knowledge of Resident Centered Care Model, APIE, and Quality Care Indicators and Outcomes. Actively participate in Sunrise Quality Assurance Meetings and Clinical Meetings. Knowledge of federal, state/provincial, and local resident care and services regulations. Understand resident rights and assessment/evaluation processes in accordance with regulatory requirements. Review physician reports of new residents as directed by the RCD. Order medications and equipment as directed by the RCD. Demonstrate knowledge of Sunrise practices for skin, nutrition, behavior, falls, and bowel/bladder care. Assist in implementation, training, and monitoring of documentation related to residents’ clinical health and wellness. Report resident changes, concerns, and solutions to the RCD as appropriate. Contribute to overall engagement programs and processes, including engagement surveys and improvement workshops. Attend regular meetings such as Town Hall, Department Team, and other departmental meetings. Maintain compliance with required training mandated by the state/province or other governing authorities. Perform other duties as assigned. Core Competencies Ability to handle multiple priorities. Proficient written and verbal communication skills, and ability to teach and guide team members. Strong organizational and time‑management skills. Good judgment, problem‑solving, and decision‑making skills. Experience and Qualifications Graduate of an approved college/school of nursing. Maintains a current state/provincial license as a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN). Minimum of one (1) year experience in home health, assisted living, hospital, or long‑term care environment. Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care and regulations. Knowledge of good assessment skills. Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Sunrise applications, with ability to learn new applications. Availability to work weekends, evenings, and flexible hours as required. Pre‑Employment Requirements Completion of a drug test (excluding marijuana), tuberculosis test, physical evaluation, and background check where required. COVID‑19 and influenza vaccination may be required if mandated by applicable laws and authorities. Location Braddock Glen, 4027 Olley Lane, Fairfax, Virginia Compensation Salary Range: USD $33.30 - $44.50 /hr. Variable Compensation: No Bonus or Commissions.
