Trans Wellness Case Manager
San Francisco Community Health Center
San Francisco Community Health Center is seeking a Case Manager to provide comprehensive case management for Trans:Thrive clients, coordinating with internal and external providers to improve health outcomes. You will conduct intakes, develop care plans, and offer health education and advocacy. The role requires experience with transgender communities, harm reduction, and strong communication skills.
