Home Health Aide (HHA)

The mission of this role Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. The HHA role is essential to that mission: you provide compassionate personal care in the home, support patients in their daily activities, and help us raise the standard for what home health can be. Objectives Carry a full, high-quality caseload Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. What You\'ll Own Provide personal care assistance including bathing, dressing, grooming, and toileting. Assist patients with safe transfers, ambulation, and mobility using proper body mechanics. Support patients with medication reminders as directed by the care plan. Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse. Perform light housekeeping tasks related to patient safety and care (laundry, meal preparation, maintaining clean environment). Document all care provided accurately and timely in Homecare Homebase (HCHB). Communicate clearly with patients, families, and the care team to ensure continuity of care. Follow infection control practices and maintain patient dignity and privacy. Assist with range of motion exercises and other restorative care activities as ordered. Coordinate with supervising RN/LVN for care plan updates and patient needs.
