MDS Nurse - RN

We are looking for an MDS Nurse‑RN to provide necessary and accurate documentation for resident and patient assessments and care plans in accordance with company standards and federal, state, and local regulations. Responsibilities Operations Management (60%) Oversee the RAI process and submission process by validating and monitoring reports (EHDS). Plan, develop, organize, and communicate assessment and care plan information to staff members to optimize resident and patient care. Oversee and complete the computer data entry and electronic submission process for MDS transmission (EHDS). Oversee Medicare and managed care logs, directing decision‑making regarding insurance coverage, cut dates, and MDS schedules. Oversee skilled resident and patient statuses to maximize reimbursement. Oversee Medicaid resident statuses to maximize case mix scores. Participate in care conferences. Remain available during surveys and audits of the facility and/or the facility’s documentation. Assist with insurance appeals by reviewing documentation coded on MDS and verifying for consistency. Attend all‑employee and departmental meetings and in‑service training as scheduled or as directed. Quality, Compliance, and Risk Management (20%) Ensure accurate and timely MDS, CAA, and Plan of Care documentation from the interdisciplinary team. Provide final review of MDS and Plan of Care documentation of all disciplines, offering education and advice as necessary and providing authorization. Maintain and communicate schedule of MDS and Plan of Care due dates. Keep up to date on changes in documentation regulations, RUG categories, and legislation affecting MDS reimbursement issues. Attend committee meetings as requested. Administration and Reporting (20%) Maximize reimbursement through team education, planning, and accurate and timely MDS assessments. Assist with reviewing and addressing issues with quality measures, survey findings, quality assurance findings, and other findings and/or survey results as assigned. Perform all other duties as assigned. Qualifications Education Registered nursing degree from an accredited college, university, or vocational school. Licensure Current unencumbered license to practice as a Registered Nurse (RN) in the state of Ohio is required. Experience Two years’ experience with MDS preferred. Two years’ experience as a nurse in a long‑term care facility required. Experience in rehabilitative and restorative nursing practices required. Knowledge Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long‑term care with a special emphasis on documentation. Proficiency Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet. Proficiency with electronic medical records (EMR) systems. Other Requirements Must be able to read, write, speak, and understand the English language.
