This position serves as a clinical pharmacist specializing in the care of critically ill and trauma patients within the Surgical Intensive Care Unit (SICU) and Trauma Intermediate Care Unit (TICU). The pharmacist works closely with physicians, advanced practice providers, nurses, and other members of the interdisciplinary healthcare team to optimize medication therapy and improve patient outcomes across the continuum of critical and acute care.
A primary responsibility of this role is active participation in daily multidisciplinary rounds at Ohio's busiest Level I Trauma Center, providing evidence-based pharmacotherapy recommendations for complex surgical, trauma, and critically ill patient populations. The pharmacist is responsible for medication therapy management, antimicrobial stewardship, therapeutic drug monitoring, medication safety initiatives, and ensuring adherence to best practices and clinical guidelines.
The position offers numerous opportunities for professional growth and leadership through involvement in quality improvement, research, medication-use evaluations, and interdisciplinary initiatives aimed at advancing patient care. The pharmacist is also expected to support the educational mission of the department through the precepting and mentoring of pharmacy students, PGY1 pharmacy residents, and PGY2 Emergency Medicine pharmacy residents.
Pharmacists at Grant Medical Center work in a progressive practice model where all pharmacists rotate through central pharmacy and 2-3 clinical areas. Pharmacy services are well integrated into the interdisciplinary teams across the campus and organization. Pharmacists in this position work a mix of day and evening shifts with every fourth weekend rotation and 2 major holidays per year commitment.
This position is responsible for dispensing and compounding medication, counseling patients on medication and health subjects; interpreting prescriptions, managing patient drug therapy for complications, and performing drug utilization. This role collaborates with physicians, nurses, and other health-care practitioners to provide safe, high quality, and cost-effective drug therapy. Pharmacists will be required to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise.

Minimum Qualifications and Desired Attributes:
Bachelor of Science or Doctor of Pharmacy degree from an accredited institution (Required)
Current Ohio pharmacist license (Required)
Completion of a PGY1 Pharmacy Residency or equivalent years of previous pharmacy experience (Required)
Completion of a PGY2 Critical Care or Emergency Medicine Pharmacy Residency or equivalent years of previous pharmacy experience (Preferred)
Pharmacy Board Certifications or other certifications
Advanced-level degree in healthcare field
Collaborative practice experience
Basic Life Support Certificationcurrent or obtained during the orientation period (unless greater than 50% of scheduled work hours, as determined by the manager, are completed off-site or not in a patient care setting)
Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager)
Specialized Knowledge:
Pharmacokinetic drug dosing experience

Responsibilities and Duties:

Acute Care/Infusion 50% - Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy-related devices in accordance with standards of practice and OhioHealth pharmacy policies. Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity. Ensures appropriate labeling for proper administration of medication. Interprets prescriptions and determines compatibility in accordance with standards of practice. Prepares care plan for patients to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.
Ambulatory Care/Population Health 50% - Manages individuals' drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with a provider. Prepares individualized care plan to ensure optimal therapeutic outcomes, avoid adverse events, and maximize adherence with therapy instructions. Provides drug information to providers, other health care professionals, and patients as needed.
Retail 50% - Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy-related devices in accordance with standards of practice and OhioHealth pharmacy policies. Ensures all medications, chemicals, and supplies are dispensed without suboptimal integrity. Ensures proper billing procedures.
Medication Safety, Stewardship, & Utilization 55% - Leads safety, stewardship, and utilization strategies for designated site(s) in collaboration with system team. Facilitates and manages activities of various site and system committees and clinical guidance councils. Develops, enforces, and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient, and appropriate medication therapy use. Utilizing data, published best practice standards, and published events or recommendations, identifies opportunities for proactive change in the medication-use system to improve patient outcomes. Integrates medication improvement initiatives, activities, and changes with clinical and operational department programs and committees. Responsible for ensuring compliance with evidence-based recommendations, national patient safety recommendations, and regulatory standards related to safe and appropriate medication use.
