Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health or Hospice experience License: Must have an active nursing license Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase and OASIS Description: Provides and coordinates the provision of home nursing care according to physician orders. Follows agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Conducts home visits and attends periodic office meetings. Must have reliable transportation and current driver’s license with agency-required liability insurance. Average number of patients per day is 5-8. Each visit and activity holds a different point value, with a target of 30 points per week. Contractor will be assigned between 5-10 patients per day. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Must be willing to travel up to 75 miles from the home clinic. Mileage is reimbursed at the IRS standard rate. All travelers must be proficient in HomeCare HomeBase EMR and OASIS within the last six months. Documentation must be completed within 24 hours of the patient appointment. Delays or incomplete documentation can impact patient care. No guaranteed hours are provided with this contract. Clinician must meet weekly productivity expectations. Overtime must be pre-approved. Must be open to occasional Saturday or Sunday shifts for time off during the week. Local candidates are accepted at the same rate. Will cover the Southern region. Must confirm awareness of responsibilities and the need for a vehicle, driver’s license, and auto insurance. Must accept every patient on the schedule regardless of travel distance. Understanding of documentation and productivity expectations is essential.
