Position Summary

NPORT is seeking a dedicated and compassionate Occupational Therapist (OT) to provide high-quality rehabilitative services at a s killed nursing and rehabilitation center in Middletown, NY . The OT will evaluate residents and develop individualized treatment plans designed to restore, reinforce, and enhance performance in activities of daily living (ADLs), functional mobility and overall independence.

Key Responsibilities
- Evaluate residents' physical, cognitive, and psychosocial functioning.
- Develop individualized treatment plans with measurable goals
- Provide skilled occupational therapy interventions in accordance with physician orders and regulatory guidelines.
- Train residents in ADLs, adaptive techniques, and use of assistive devices
- Collaborate with interdisciplinary team members to coordinate care
- Educate residents, families, and caregivers regarding therapy plans and progress.
- Maintain accurate and timely documentation in compliance with state, federal, and facility requirements.
- Ensure compliance with Medicare, Medicaid, and third-party payer guidelines.

Qualifications
- Graduate of an accredited Occupational Therapy program — Current New York State OT license in good standing
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Experience in skilled nursing or rehabilitation settings preferred

Physical Requirements
- Ability to lift, transfer, and assist residents as needed
- Ability to stand, walk, bend, and perform repetitive tasks throughout the workday.
