Employment Type Part time Shift Day Shift Location Trinity Health Livonia, Livonia, MI Job Overview An advanced level nursing role as a Case Manager within Trinity Health Livonia. The ideal candidate will assess patient needs, facilitate inpatient planning using criteria sets, and collaborate with physicians and other healthcare providers to develop safe and appropriate plans of care for patients. Case Managers may work in the Emergency/ICU, Birthing Center, or medical/surgical floors with either inpatients or Observation patients depending on assignment. Responsibilities Assess patient needs and manage disease progression, critical pathways, and continuum of care from admission through discharge. Facilitate inpatient planning using standardized criteria and collaborate with physicians and other healthcare providers to develop safe care plans. Work within the Emergency/ICU, Birthing Center, or medical/surgical floors with inpatients or Observation patients. Qualifications Minimum education: Bachelor of Science in Nursing or related health field, or equivalent work experience. Current nursing licensure in Michigan. Progression toward or completed certification in case management required. Current Basic Life Support (BLS) certification. Minimum five years of clinical nursing experience and recent (within 2 years) experience in utilization review, discharge planning, or case management. Knowledge of third‑party payor programs, requirements, and applied criteria. Familiarity with federal, state, and local regulations affecting utilization review programs and payments. Understanding of regulatory and accrediting agency standards related to utilization review. Benefits Competitive compensation.
