Program Specialist I
The Program Specialist I is responsible for coordinating and providing individualized, person-centered supports for individuals with intellectual and developmental disabilities and other related needs. This position ensures individuals receive the services, resources, and supports necessary to live as independently and successfully as possible in their homes and communities.
The Program Specialist I conducts assessments, develops and monitors Individual Support Plans (ISPs), coordinates medical and community-based services, and serves as a key liaison among individuals served, families, guardians, staff, and external service providers. The role focuses on promoting health, safety, community inclusion, personal growth, and overall quality of life through effective service coordination and program oversight.
Program Oversight & Quality Assurance
- Ensure services are delivered in a professional, respectful, and person-centered manner.
- Monitor program operations and service delivery to ensure quality outcomes.
- Ensure compliance with agency policies, licensing standards, funding requirements, and applicable regulations.
- Evaluate program effectiveness and identify opportunities for improvement.
- Ensure adequate resources, equipment, and supplies are available to support individuals and program operations.
- Collaborate with admissions and discharge teams regarding individuals requiring residential or supportive living services.
- Provide guidance and support to staff regarding person-centered practices and service implementation.
Individual Support Planning & Case Management
- Assess individual needs and develop person-centered service strategies.
- Coordinate the development, implementation, and monitoring of Individual Support Plans (ISPs).
- Ensure individuals receive appropriate services based on their goals, preferences, and support needs.
- Maintain accurate, confidential, and up-to-date case records and documentation.
- Monitor progress toward individual goals and recommend modifications when necessary.
- Coordinate services among internal departments and external providers.
- Consult regularly with families, guardians, and support networks to ensure continuity of care.
Medical & Service Coordination
- Coordinate and schedule medical appointments and related healthcare services.
- Monitor follow-through on medical recommendations, treatment plans, and prescribed services.
- Communicate healthcare information and updates to staff, caregivers, and appropriate stakeholders.
- Collaborate with healthcare professionals to support positive outcomes for individuals served.
- Ensure service providers have accurate and current information regarding individual needs and supports.
Community Resource Development
- Develop and maintain strong partnerships with community organizations and service providers.
- Assist individuals in accessing community resources, benefits, recreational opportunities, employment supports, and day programs.
- Promote community integration, engagement, and inclusion.
- Educate individuals, families, and support teams regarding available services and resources.
Individual & Home-Based Support
- Provide person-centered support in residential and community-based settings.
- Assist with identifying and addressing daily living support needs, including:
- Personal care and hygiene
- Household management
- Independent living skills
- Community participation
- Respond appropriately to changing needs, emergencies, or unexpected situations.
- Advocate for individual choice, independence, and self-determination.
Communication & Collaboration
- Serve as a liaison between individuals served, staff, families, guardians, healthcare providers, and community partners.
- Facilitate communication among all members of the support team.
- Address concerns, service issues, and support needs promptly and professionally.
- Participate in team meetings, interdisciplinary planning sessions, and agency committees as required.
Administrative Responsibilities
- Maintain confidential files, reports, and documentation in accordance with agency policies and HIPAA requirements.
- Prepare reports, assessments, and program documentation as required.
- Participate in program planning, quality improvement initiatives, and resource allocation activities.
- Complete assigned administrative tasks accurately and within established deadlines.
- Perform additional duties as assigned.
Qualifications
Education
- High School Diploma or GED required.
- Bachelor's degree in Human Services, Social Work, Psychology, Special Education, Rehabilitation, or a related field preferred.
Experience
- Minimum of three (3) to five (5) years of experience working in:
- Intellectual and Developmental Disabilities (IDD) services
- Human services programs
- Behavioral health services
- Residential or community-based support programs
- Related healthcare or social service settings
Knowledge, Skills & Abilities
- Strong understanding of person-centered planning and service coordination.
- Knowledge of Individual Support Plans (ISPs) and community-based support services.
- Excellent organizational, documentation, and time-management skills.
- Strong analytical, problem-solving, and decision-making abilities.
- Ability to work independently with minimal supervision.
- Effective interpersonal, written, and verbal communication skills.
- Ability to build collaborative relationships with individuals, families, providers, and community partners.
- Proficiency in Microsoft Office applications and electronic documentation systems.

- Maintain compliance with all agency policies, procedures, and professional standards.
- Adhere to HIPAA confidentiality requirements and privacy regulations.
- Comply with OSHA safety standards and workplace safety policies.
- Attend and successfully complete all required trainings and meetings.
- Maintain accurate records of work activities, documentation, and time reporting.
- Possess a valid driver's license and maintain an acceptable driving record, if required by the position.

- Sit, stand, walk, bend, reach, and perform routine physical activities throughout the workday.
- Travel to program locations, community settings, and participant residences.
- Safely operate a motor vehicle when required.
