Job Title: RN Profession: Nursing Specialty: Home Health Duration: 260 Days Shift: 5x5 Days Hours per Shift: Not specified Experience: Two (2) years in a home health setting within the last ten (10) years License: Current RN Licensure in Client Certifications: BLS (required before the start date) Must-Have: - Two (2) years of experience in a home health setting within the last ten (10) years - Recent experience conducting comprehensive assessments of patients - Experience caring for a minimum of two patients per week Description: Required licensure: Current RN licensure in Client. Required experience: Two years of experience in a home health setting within the last ten years. Nurses must have recent experience conducting comprehensive assessments of patients. Experience may include working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Conducting assessments that are similar to the UAS such as OASIS is preferred. Providing primary care to homebound persons is essential. Care management for persons receiving home and community-based services is required. Home care experience as a paraprofessional or Licensed Practical Nurse is considered valuable. Preferred qualifications include experience conducting assessments in UAS-NY, especially in the last 365 days. Experience managing chronic diseases in a geriatric population is preferred. Candidates must be prepared to conduct hybrid tele and in-field assessments. Ability to review health status questions and make evaluations based on UAS criteria is necessary. Strong knowledge of nursing principles, state laws, and regulations governing nursing practices is essential. Excellent organizational, interpersonal, written, and verbal communication skills are required. Candidates must demonstrate sensitivity to the concerns of others. Ability to work effectively in a fast-paced, deadline-oriented environment is necessary. The capacity to manage multiple complex tasks simultaneously is essential. Candidates should work independently and take initiative. Responsibilities include performing initial in-home evaluations of patients. Evaluating health status, strengths, care needs, and preferences is essential. Reviewing consumer medical documentation as relevant to each case is required. Entering evaluation data into electronic forms and transmitting as required is necessary. Documentation of concerns and other issues during the evaluation process is critical. Continuity of care should be emphasized to reduce fragmentation and duplication. Discussing healthcare options and supports with consumers is required. Initiating communication between consumers and enrollment processes is essential. Enhancing communication with interdisciplinary care team members is necessary. Collecting quality review data and documenting case notes into the relevant system is required. Identifying opportunities for health promotion and illness prevention is critical. Maintaining knowledge of community resources and services for the target population is essential. Meeting all established performance standards and criteria is expected. Performing additional duties as assigned by management is required.
