Nurse Practitioner or Physician Assistant Geriatric Pain Management

A growing pain management practice is hiring a Nurse Practitioner or Physician Assistant to provide specialized pain and palliative care to geriatric residents in skilled nursing facilities across the Sacramento area. You'd round on the same residents week after week, conducting comprehensive pain assessments, building individualized treatment plans, managing medications, and coordinating with facility staff and physicians. Key Responsibilities Conduct comprehensive pain assessments: medical history review, physical exams, and identifying the source and level of pain. Develop individualized, evidence-based treatment plans for acute and chronic pain, combining pharmacological, non-pharmacological, and interventional approaches. Prescribe and manage pain medications, monitoring dosages and maintaining responsible prescribing practices per state and federal regulations. Refer to and coordinate additional therapies, including physical therapy, acupuncture, spinal blocks, and other interventional techniques. Educate patients and families on pain management strategies, coping mechanisms, and lifestyle modification. Coordinate care closely with physicians, physical therapists, and other providers to optimize outcomes. Monitor treatment effectiveness through follow-up visits and adjust interventions as needed. Maintain accurate, timely records in TEBRA and PointClickCare. Build strong working relationships with skilled nursing facility leadership and staff. Required Qualifications Active, unrestricted California NP or PA license. AGACNP- BC, AGPCNP-BC, FNP-BC or PA-C certification. Willing to work fully onsite at skilled nursing facilities — this role cannot be done remotely or hybrid. Able to travel between facilities in the Sacramento area. Strong communication skills, compassion, and critical thinking, with a commitment to staying current in the field. Ability to build and maintain professional relationships with facility leadership and staff. Preferred Qualifications Prior experience caring for geriatric patients. Background in skilled nursing, long-term care, palliative care, or hospice. Solid understanding of pain mechanisms, pain management pharmacology, and the psychosocial aspects of pain. Experience with TEBRA or PointClickCare. Compensation and Benefits $60–$100 per hour depending on experience, plus a productivity bonus. Malpractice insurance, including tail coverage.
