Nurse Practitioner

Job Summary Part‑time Nurse Practitioner – In‑Home Health Evaluations at Signify Health, part of CVS Health®. The role delivers 45‑minute personalized healthcare visits in patients’ homes, utilizing an iPad and a streamlined clinical workflow. The clinician focuses on assessment, medication & medical history review, and diagnostic testing as ordered by the patient’s health plan. Prescribing and lab ordering are not required. Visit millions of people nationwide, connecting them to the right care for their unique needs. Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role. Be flexible to travel locally and within licensed states, as needed. Conduct virtual visits as required. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams). Use an iPad and an intuitive clinical workflow for each evaluation. Perform medical history review, physical evaluation, and optional diagnostic tests provided by the health plan. Qualifications & Licensure Active, unrestricted medical license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Anticipated weekly hours: 29.5. Part‑time employment. Pay Typical pay range: $37.66 - $81.11 per hour. Base salary will be determined based on experience, education, geography, and other factors.
