Home-Based Family Model Caregiver (Contractor)

RHA Health Services is seeking compassionate Family Model Providers in Missouri to support adults and children with I/DD at home. As an independent contractor, you offer a safe, comfortable living environment and work under a multidisciplinary team. Responsibilities include understanding the person’s history, assisting with meals, self-care, transportation, medical appointments, and community participation, while honoring choices and privacy.
