Job Description
Registered Nurse - Case Manager

Schedule

Day shift, 5 days per week (08:00 - 16:30). Required to work every other weekend.

Assignment Details

Start date: ASAP. Floating between like units may be required based on need.

Requirements

• 2 years of acute care experience required.
• First-timers accepted with clinical approval.
• Must have a valid nursing license.
• Experience with Epic EMR system preferred.

Responsibilities

• Coordinate patient care and services across the continuum.
• Assess patient needs and develop care plans.
• Facilitate communication between healthcare providers, patients, and families.
• Monitor patient progress and modify treatment plans as necessary.
• Document patient interactions and care provided.
