Americare Inc. seeks an RN Intake Hospital Liaison to coordinate the intake process for patients referred to home care services. You will act as the primary liaison between hospitals, referral sources, patients, families, and internal teams to ensure a smooth transition from hospital to home. Requirements include a current NYS RN license, experience with Allscripts and CHHA, and proficiency with Home Care Home Base; strong communication, organization, and the ability to work under pressure are #J-18808-Ljbffr Americare Inc.
