You will act as the primary liaison between hospitals, referral sources, patients, families, and internal teams to ensure a smooth transition from hospital to home. Requirements include a current NYS RN license, experience with Allscripts and CHHA, and proficiency with Home Care Home Base; strong communication, organization, and the ability to work under pressure are #J-18808-Ljbffr Americare Inc.
