Mental Health Case Worker

The following statements are intended to describe the general nature and level of work being performed. Position Summary: A Mental Health Worker/Case Manager will assist in the development of treatment plan(s) that are appropriate to the needs and desires of the client. The treatment plan(s) will aim to promote recovery and stability for community living and helping the client to achieve higher levels of function. Program Information: The Housing Supportive Services Program (HSSP) is a specialized program that provides comprehensive and intensive field-based community mental health services designed to meet the unique needs of individuals. HSSP services are provided as part of an integrated model in which the Los Angeles County (LAC or County) Department of Mental Health (Department or DMH), Department of Health Services (DHS), and Department of Public Health (DPH) partner to respectively provide mental health services, intensive case management services (ICMS) and substance use services, as appropriate, to clients who are in or have been matched to PSH. The integration of services provided to clients is expected to decrease incarcerations, homelessness, hospitalizations, and frequent visits to psychiatric emergency rooms. Core Duties And Responsibilities: Monitor client compliance with treatment as prescribed by treatment team in the community.Develop individualized case management treatment plan(s) with the client.Assist with establishing coordination with local agencies in the provision of services.Assist with the development and maintain resource directory for agency.Update and maintain client records.Arrange for medical care-exams, emergency shelter, food and/or education services for clients at large.Facilitate support/self-help groups for clients, family members and the community at large.Participate in case disposition meetings on a routine basis with the assigned treatment team.Provide and document services in accordance with the Rehabilitation Option/Targeted Case Management requirements.Participate in the Mental Health education program provided to the community.Coordinate client care with other providers both within and outside the agency.Attend and participate in all in-service training as necessary.Responsible for timely completion of documentation in the clients clinical record per Enki, LA County Department of Mental Health and legal requirements.Responsible for meeting assigned minimum productivity requirements on a monthly basis.Other duties as assigned.Comply with all Company policies and procedures as applicable.
