Social Worker – Palliative Care

Trellis Supportive Care is seeking a compassionate and dedicated Part-time Social Worker, MSW – Palliative Care to provide skilled medical social work services to palliative care patients and their families.

Position Details

Schedule: Part-Time 20 hours per week

What You’ll Do

As a Palliative Care Social Worker, MSW you will provide individualized social work services to patients and families while helping them navigate the emotional, social, financial, and practical challenges associated with serious and life-limiting illness.

You will:

• Assess patients’ and families’ emotional, social, and financial needs and develop appropriate goals and interventions.
• Initiate and support end-of-life conversations while helping patients and families understand and cope with the dying process.
• Provide skilled social work services to palliative care referrals and patients.
• Treat the patient and family as the unit of care while honoring each individual’s right to dignity, respect, and compassionate support.
• Provide ongoing support and communication in patients’ homes, hospitals, inpatient settings, and long-term care facilities as needed.
• Collaborate with physicians and other members of the interdisciplinary team to address complex patient and family needs.
• Connect patients and families with appropriate community, financial, healthcare, grief, and other supportive resources.
• Assist with Advanced Directives, end-of-life needs, complementary therapies, discharge planning, and other resources.
• Provide consultation, support, and guidance to members of the care team as needed.
• Explain available benefits, fee-for-service arrangements, and payment mechanisms, including Medicare and Medicaid benefits when applicable.
• Complete financial assessments and required reimbursement documentation as appropriate.

What You Bring

Education & Experience

• Master’s degree in Social Work required.
• LCSW preferred.
• Minimum of one (1) year of recent medical social work experience required.

• Exceptional documentation and organizational skills.
• Compassionate and effective interpersonal communication skills, both verbal and written.
• Knowledge of community social, financial, and healthcare resources.
• Understanding of the medical aspects of caring for patients with life-limiting illness and the psychosocial impact on patients and families.
• Working knowledge and understanding of palliative care techniques and commitment to the philosophy of hospice and palliative care.
• Computer proficiency, including electronic medical records and Microsoft 365
• The ability to establish effective working relationships within an interdisciplinary team with minimal supervision.
• Flexibility to adjust schedules as needed to meet patient and family needs.
