Experience / Requirements

• Master's degree
• Nurse Practitioner with National Board Certification and active state license
• 1+ year recent Primary Care NP experience preferred (or completion of CVS MinuteClinic training)
• Knowledge of chronic disease management and quality measures
• Strong communication, organizational, and problem-solving skills
• Ability to work independently and collaboratively in a clinical team
• Proficient with clinical technology and information systems
• Willing to obtain multi-state licensure and work remotely when required

Duties / Responsibilities

• Provide evidence-based primary and family care including assessment, diagnosis, and treatment
• Manage patient health plans, chronic conditions, and preventive care
• Perform routine exams and coordinate care with healthcare teams
• Handle clinic operations, patient follow-ups, and administrative tasks
• Educate patients on healthy lifestyle and wellness programs
