Patient Services Coordinator

To support patient care initiatives, the full-time Patient Services Coordinator will manage telephonic outreach, medication scheduling, and data analysis while working in a hybrid environment of two days onsite and three days remote in New Providence, NJ. Key responsibilities Conduct telephonic outreach for patient education, adherence support, and retention Schedule medication and supply deliveries while collaborating with nursing and outside agencies Analyze reports and participate in retention and discharge committee meetings to ensure program success Required qualifications High school diploma or equivalent; Bachelor's degree in healthcare or business preferred Strong data analysis and organizational skills Experience with patient admissions in a referral source-based practice setting preferred Customer service experience in a Pharmacy Specialty or healthcare services business is strongly preferred Previous experience working with infusion therapy patients is highly desirable
